Bengaluru: Cognizant subsidiary TriZetto Provider Solutions (TPS) has suffered a data breach that exposed the sensitive information of about 3.4 million individuals, according to a filing with the Office of the Maine Attorney General.
TPS said it has begun notifying affected individuals about the cybersecurity incident, which may have exposed certain protected health information.
According to the company, suspicious activity was detected on October 2 last year within a web portal used by some of its healthcare provider clients to access TPS systems. The company said it launched an investigation immediately after identifying the activity, engaged external cybersecurity experts and informed law enforcement authorities. The probe found that an unauthorized actor may have accessed certain records beginning in November 2024. The compromised records were linked to insurance eligibility verification transactions used by healthcare providers to determine insurance coverage for patient treatment.
TPS said the exposed data may include individuals’ names, addresses, dates of birth, social security numbers, health insurance member numbers, provider names, health insurer names, primary insured details, and other demographic and health-related information.
The company said the breach did not involve payment card data, bank account details or other financial account information. It also noted that it is not aware of any cases of identity theft or fraud linked to the incident so far. TPS said it has strengthened its security systems following the breach and implemented additional protective measures. The company began notifying affected healthcare providers on December 9 last year. To support those impacted, TPS is offering complimentary credit monitoring, credit report and credit score services through Kroll for a limited period.
Responding to queries from TOI, Cognizant said, “In October 2025, TPS became aware of suspicious activity within a web portal used by some of TPS’s US healthcare provider customers to access our systems. We engaged external cybersecurity experts, eliminated the threat and notified affected customers and patients, providing them with the necessary support and information.”
